When a client calls in to make an appointment to obtain a health certificate please advise the following:

• Have complete information on person traveling with pet ie name, phone number and current address.
• Have complete address with phone number of destination
• Have copy of any previous vaccinations and health records if possible.
• Client is responsible for obtaining international regulations for travel. They can obtain that by either calling (804)771-2774 USDA Federal Office of Vet.Services or they can go online.

Distributions of health certificates:

We are to keep our copy in client's chart and give the remaining 5 copies to client. They are responsible for mailing to USDA VS along with fee (depending on what country). Client can call above number to find out details of fees or go online.
